Impact Testing
Will be scheduled by our training staff at a later date and time. They will directly contact coaches to schedule a date and time for impact testing to be completed. Our impact testing coordinator is Tori Ruvolis.
Physicals
Physicals will be conducted by LVHN at their various locations. Please see below for instructions:

The Winter Sports season starts on 11/18. We would like all physicals to be in our office by 11/8!

Sections Needed for Physicals/Recertification Forms:
Sections 1 through 7 are needed if the student-athlete did not play a fall sport. Section 8 (ONLY) is needed if the student-athlete played a fall sport. Section 9 will also be needed for those student-athletes that marked a “no” answer on Section 8 on the physical form.
