LMMS Families –<\/p>\n\n\n\n

The district medication drop off day is Wed 8\/13 from 8-2. <\/strong><\/p>\n\n\n\n

For parent convenience and to avoid the business of the first day of school, nurses will be available district wide to accept medications for the upcoming school year. <\/p>\n\n\n\n

Over the counter medications need to be in the original container with the medication authorization form signed by the parent\/guardian.<\/p>\n\n\n\n

All prescription medications need to be in the original container, labeled by the pharmacy and be accompanied with the medication authorization form signed by both the parent\/guardian and provider.<\/p>\n\n\n\n

All medications must be brought to LMMS by an adult and given directly to our school nurse. <\/p>\n\n\n\n
