A report should be issued for the student if he/she is in the program long enough to bridge a report period. This report will be developed by a collaborative effort of the homebound instructor, counselor and the regular classroom teacher(s).
A Homebound Instruction Report must be completed on a monthly basis for any student enrolled in a homebound program. If the student completes the program in less than a month, the homebound instructor will use the Homebound Instruction Report to do a final report. This report is to be completed in full and given to the elementary principal or the appropriate secondary counselor.
A homebound instructor working with a secondary student (grades 6-12) will be given a maximum of one (1) hour per week of planning/contact time with classroom teacher(s) per core subject area taught. A homebound instructor working with an elementary student (grades K-5) will receive a maximum of one (1) hour per week of planning/contact time with the student’s designated elementary teachers. The Office of Student Services will apply these hours to the Homebound Instruction – Verification / Timesheet form.
Maintenance of Records:
The administrative assistant to the Director of Student Services shall be responsible for maintaining referrals for students and timesheets for homebound instructors. The administrative assistant will provide the necessary forms to the homebound instructors. An end-of-the-year report will be completed for submission with the district’s Attendance Report.
