August 13th from 8am to 2pm- The School Nurse will be at Willow Lane to accept medication for the upcoming school year. Over-the-counter medications must be accompanied by a “Medication Authorization” form signed by a parent/guardian. A prescription medication must be accompanied by a “Medication Authorization” form (or equivalent) signed by the parent/guardian and prescriber ; an after-visit summary that lists the details of the medication is generally acceptable for the prescriber portion. The medication must be presented in the original labeled container.
