Medication Drop-Off Procedures

LMMS Families –

The district medication drop off day is Wed 8/13 from 8-2.

For parent convenience and to avoid the business of the first day of school, nurses will be available district wide to accept medications for the upcoming school year.

Over the counter medications need to be in the original container with the medication authorization form signed by the parent/guardian.

All prescription medications need to be in the original container, labeled by the pharmacy and be accompanied with the medication authorization form signed by both the parent/guardian and provider.

All medications must be brought to LMMS by an adult and given directly to our school nurse.
