Role of the Home and School Visitor

In order to minimize barriers to learning and maximize the student’s ability to have success at home, in school and in the community, the role of the Home and School Visitor is as follows:
- Consult and collaborate with school personnel to identify students who are encountering significant barriers to learning and work to link students/families to school and community-based resources.
- Serve as liaison between students, parents, teachers, counselors and community agencies
- Provide families with information and assistance to access appropriate community-based resources (MH/MR, Medical Assistance, financial assistance, etc.)
- Case management of student cases
- Facilitate child specific interagency/problem solving meetings
- Participate in child specific school/agency meetings in order to coordinate services and establish effective communication and cooperation between systems.
- Create positive working relationships with community-based agencies for the purpose of efficiently utilizing these services for students and their families
- Provide staff development regarding current community resources, eligibility and how to access services
- Act as Homeless and Migrant student coordinator for PA Dept of Education
- Develop parent support and education programs
- Collaborate with school personnel to design a district-wide plan for early intervention for “at-risk” students
- Collaborate with school personnel to design a district-wide truancy prevention/intervention program
- Build the school district’s capacity by creating partnerships with business and community stakeholders.
