Required Health Exams

Pennsylvania School law requires specific health exams at specific grade levels for attendance in school. Please refer to the following chart:

If your student is entering this
grade in Fall 20/21:
This health exam is required:

You may choose for this exam to be done by your private provider or by the school provider. The required examination may be done anytime  after July 1, 2019. Once the private examination is completed, return the exam form to your school nurse. 

The following health exams are mandated by the Pennsylvania Department of Health: 

Physical Exams

  • A physical examination is required upon original entry into school and again in 6th and 11th grades.
  • It is recommended that this exam be performed by your private physician who is more familiar with your child’s medical history.
  • The required private physical examination form can be obtained from your child’s school nurse or downloaded Report of Physical Examination or Report of Physical Examination (Spanish). The private examination may be done within one year from the start of the current school year. Once the private examination is completed, return the private physical form to your school nurse.
  •  If you decide to have the physical exam performed by the school physician, a School Physical permission form must be completed and submitted to the school nurse. You will be notified of the date and time that the school physician will be performing the physical examination.

Dental Exams

  • A dental examination is required upon original entry into school and again in 3rd and 7th grades.
  • The required private dental examination form can be obtained from your child’s school nurse or download Report of Dental Examination or Report of Dental Examination(Spanish). The private examination may be done within one year from the start of the current school year. Once the private examination is completed, return the private dental form to your school nurse.
  •  If you decide to have the dental exam performed in school, a School Dental permission form must be completed and submitted to the school nurse. You will be notified when the school dentist will be performing the dental examination.

If the required exam is not submitted on or around the end of January of the current school  year, your child may be excluded from school until the necessary report of examination is submitted. Please refer to EPSD Board Policy on Health Exams (#209).
