PIAA Physical Information

All in-season paperwork for Student-Athletes MUST be completed and submitted ON-LINE. Below please find the set of instructions that pertains to you and link to submit your student-athletes paperwork.

FIRST TIME completing Student Athlete paperwork for the current school year – Find step by step instructions on completing the process in the BIG TEAMS East Penn School District – student central letter. This link is an informational letter which includes the PIAA Physical form and the East Penn School District Athletic Department Forms.
If paperwork PREVIOUSLY submitted and approved in the current school year – Find step by step instructions on completing the recertification process in the BIG TEAMS East Penn School District – recertification directions.
- Section 8 (if needed) – Re-Certification by Licensed Physician of Medicine or Osteopathic Medicine – The form must be completed for any student who, subsequent to completion of Sections 1 through 5 of the CIPPE form, required medical treatment from a licensed physician of medicine or osteopathic medicine.

PIAA Examination Information

Scheduling your Student Athlete’s PIAA examination

The Lehigh Valley Health Network is offering free PIAA Comprehensive physical examination. If you are in need of this, the window for self scheduling through the LVHN will open on May 1st 2025 and close on August 10, 2025. NO appointments will be scheduled prior to May 1. LVHN instructions can be found using this link along with the PIAA Comprehensive Physical form. Feel free to print pages 5 and 6 to take to the Doctor appointment.

Sections Needed for Physicals – PLEASE READ CAREFULLY

Any student athlete that has participated in or tried out for a sport in the 25-26 year does NOT need a new full physical. They WILL need to complete the following sections of the physical:

All Fall and Winter Athletes, and anyone who tried out, will need to complete only SECTION 7: RE-CERTIFICATION BY PARENT/GUARDIAN of the PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION.
If any of the answers are “YES” to the questions in Section 7 you will need to complete Section 8 also: Re-CERTIFICATION BY LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE.”

Any student athlete that did not play a fall or winter sport will need a full physical, which are sections 1-6 of the PIAA comprehensive form.
