Scheduling your Student Athlete’s PIAA physical
Please mail your completed physical / recertification to:

Or please return physicals for middle school and high school athletes to the Emmaus HS Athletic Office (Enter at the Main Office between 8:00am-2:00pm/Monday-Friday.)
Sections Needed for Physicals – PLEASE READ CAREFULLY
Any student athlete that has participated in or tried out for a fall or winter sport this year does NOT need a new full physical. They WILL need to complete the following sections of the physical:
- All Fall and Winter Athletes, and anyone who tried out, will need to complete only SECTION 7: RE-CERTIFICATION BY PARENT/GUARDIAN of the PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION.
- If any of the answers are “YES” to the questions in Section 7 you will need to complete Section 8 also: Re-CERTIFICATION BY LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE.”
Any student-athlete that had a full PIAA physical between June 1st 2023-December 31st 2023 will be the same as the 2 bulleted points above
Any student athlete that did not play a fall or winter sport will need a full physical, which are sections 1-6 of the PIAA comprehensive form.
Full Physical and Re-certification Forms Due Date: 2/21/24
