2024 Fall Sports Sign-Ups
PIAA Physical Information – NEW for the 2024-2025 School Year
All in-season paperwork for Student-Athletes MUST be completed and submitted ON-LINE. Please use this link to submit your student athlete’s information. Find step by step instructions on completing the process in the BIG TEAMS East Penn School District – student central letter. This link is an informational letter which includes the PIAA Physical form and the East Penn School District Athletic Department Forms.
Scheduling your Student Athlete’s PIAA physical
The Lehigh Valley Health Network is offering free PIAA Comprehensive physicals. If you are in need of this, the window for self scheduling through the LVHN will open on 5/27/24 and close on 8/9/24. NO appointments will be scheduled prior to 5/27/24. LVHN instructions can be found using this link along with the PIAA Comprehensive Physical form. Feel free to print pages 5 and 6 to take to the Doctor appointment.
Sections Needed for Physicals – PLEASE READ CAREFULLY
Any student athlete that has participated in or tried out for a fall or winter sport this year does NOT need a new full physical. They WILL need to complete the following sections of the physical:
- All Fall and Winter Athletes, and anyone who tried out, will need to complete only SECTION 7: RE-CERTIFICATION BY PARENT/GUARDIAN of the PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION.
- If any of the answers are “YES” to the questions in Section 7 you will need to complete Section 8 also: Re-CERTIFICATION BY LICENSED PHYSICIAN OF MEDICINE OR OSTEOPATHIC MEDICINE.”
Any student-athlete that had a full PIAA physical between June 1st 2023-December 31st 2023 will be the same as the 2 bulleted points above
Any student athlete that did not play a fall or winter sport will need a full physical, which are sections 1-6 of the PIAA comprehensive form.
