PIAA Examination Information

Scheduling your Student Athlete’s PIAA examination

The Lehigh Valley Health Network is offering free PIAA Comprehensive physical examination.  If you are in need of this, the window for self scheduling through the LVHN will open on May 1st, 2026 and close on August 21st, 2026.  NO appointments will be scheduled prior to May 1st or after August 21st.  LVHN instructions can be found using this link along with the PIAA Comprehensive Physical form. Print pages 5 and 6 to take to the Doctor appointment.

**New for this session**

Student athletes who have a current annual well check on file with their LVHN primary care clinician can have their PIAA physical paperwork signed by their clinician at no charge during the designated physical window dates. The PIAA pre-participation physical cannot and should not serve as a replacement for an annual well check and LVHN strongly encourages all families to stay current with their children’s annual well checks.
